We advise reading through our FAQs before filling in our application form.
Please note: For an application to be reviewed, the person seeking one of our dogs must personally complete their own application form (or with the assistance of a writer). Applications written ‘on behalf of’ will not be considered.
If you have a disability and believe a Mobility Dog will be beneficial to helping you live your life then please apply via one of the methods below.
The application process is explained within the form. You can fill in the application form via the online link below or alternatively you can print out the pdf, fill it in and scan and email it back to gemma.overton@mobilitydogs.co.nz or return via mail to Mobility Dogs, PO Box 84031, Westgate, Auckland 0657.
If you are looking for a Mobility Dog to work with you in a professional capacity in a therapeutic environment, or for a Mobility Dog to support a person within a family unit then please contact gemma.overton@mobilitydogs.co.nz to find out more about these dogs.
